The on call emergency physician will care for you at the hospital. If you go to the ER always bring a copy of your current prescriptions.
If you have forms or paper work that need to be filled out please leave them with the front desk staff with directions for the doctor. Make sure to complete your portion of the form. Please note that forms may take 1-2 weeks for completion. The front desk should be able to inform you of the approximate fee for any forms/paperwork.
Non OHIP Covered Services
There are several services family doctors provide that are not covered by OHIP and that will be billed directly to the patient.
A list of these services (e.g. Sick notes, back to work notes, form completion etc.) and their corresponding fees, which are based on provincial standards, is posted in each exam room and is available upon request.
Removal of certain skin lesions including warts (except those on feet and genitals), are considered cosmetic. There is a charge associated with these procedures. We will inform you of your fees in advance of all non-covered visits and procedures.
This Clinic provides a clinical education setting for medical learners such as medical students and Residents from the various medical schools in Ontario. Occasionally, medical learners may participate in your care while being supervised by your doctor and we hope that you will welcome them during their time with us.
All visits are made by appointment only, there will be NO walk in appointments. Telephones are answered at 8:30 am Monday to Friday.
Please indicate to the front staff the reason for your appointment, so the appropriate time can be given for your appointment. We will always honour and respect your privacy and concerns re: confidentiality.
The physicians work varied hours. We will make every effort to have you seen by your own physician but if she/he is away our clinic provides after hour coverage for urgent issues, so please call the clinic first before going to a walk-in clinic.
Please honour your appointment times and notify the Clinic of any cancellations at least 24 hours in advance. Failure to do so will result in a financial charge of $40-80 depending on appointment type. The phones accept cancellations 24 hours a day for your convenience.
Periodic Health Exam
Periodic health examinations (formerly referred to as your yearly physical) must be booked in advance as this visit will be dedicated to do a comprehensive health exam and education session focusing on preventative care and screening.
You should have one every 1-2 years. Speak to your physician on how frequently you require this. These appointments are dedicated to screening (such as mammograms & colon cancer screening) and preventative health discussions, so if you have urgent issues that need to be discussed, please book a separate appointment.
The policy in our clinic is to have all children immunized (unless there are allergies), which is in accordance with all current practice guidelines provided by the Canadian Pediatric Society. The physicians value the importance of immunization in the care of your children, but if you have questions or concerns please don't hesitate to ask.
Please inform the front staff if you DO NOT want message left at the home contact number in your chart.
Should your situation change please be sure to keep them informed.
Keeping your Medical File Complete
Every time you are seen by another health care professional (eg. ER physician, specialist, physiothera-pist, dietician, chiropractor), please ensure a copy of their note and all investigations is sent to our office for our records. This will help ensure that you receive the best possible care.

You are responsible for making sure you see the doctor before your medications run out. Some medications require regular blood work and follow-up visits for your safety. If you do not do the required blood work and attend appropriate follow- up visits then your doctor may not be able to continue prescribing the medication as this may put your health at risk.
